Patients with IP, RA or PsA ie not just RA ; . All patients however did attend hospital within 12 months of registration. It is important to note that only 48% of the patients were treated with DMARDs or steroids over the 5 year follow-up. Therefore over half the patients were considered to have disease that did not require `aggressive' treatment. It should be noted that patients with higher HAQ scores were more likely to be treated with DMARDs Table 4 ; The HAQ distribution shows that 45.2% of patients had scores 1 and 65.2% 1.5. It is unlikely that patients with scores 1 would be considered for biologics. The model would be greatly improved if only data on patients who received DMARDs or steroids was used. In addition decisions on selection of a biologic should only be made if a HAQ threshold, say 1.5, was reached. This would more closely reflect clinical decision making. Patients on the BSR Biologics Register who were treated with etanercept had a baseline mean HAQ score of 2.1 SD 0.5 ; . Section 4.2.2 p.185 Table 36 Data used in the BRAM Given the HAQ distribution it is not surprising that the ICERs are higher than those obtained in the Wyeth model as patients with mild disease with limited room for utility gain would be treated with expensive drugs. This would be particularly true 1st line. It can be concluded that BRAM did not model the licensed indication for RA patients prior to MTX use, that is patients with severe, active and progressive disease. In addition it should be noted that a standard HAQ progression rate of 0.033 pa has been used but in the NOAR data set, the untreated group did not change over 5 years, and those treated within 6 months had a decline in median HAQ from 1.25 to 0.875. Data used in the BRAM -HAQ changes on treatment Data on etanercept that now extends to 8 years show that in the long term there is no increase in HAQ, unlike treatment with traditional DMARDs!

Women with adrenal insufficiency suffer from pronounced impairment of well-being and mood 43 ; , and DHEA replacement in these patients has been shown to improve libido 41 ; as well as mood and well-being 41, 44, 45 ; . Table 3 summarizes the published studies employing DHEA therapy in women. By contrast, women with physiological menopause and intact adrenals did not show improvement in mood or self-perceived well-being following DHEA therapy 46 ; . In general, the effects of DHEA replacement on mood may be more complex than those of testosterone replacement, as DHEA also potentially exerts direct neurosteroidal effects at gamma-aminobutyric acid and N-methyl-D-aspartate receptors, suggestive of a potential anti-depressive action 39 ; , which has been demonstrated in women with midlife-onset dysthymia, significantly benefiting from DHEA replacement 47, 48 ; . In the largest study published on the use of DHEA in elderly women 60 to 79 years old ; , comparing 12 months of treatment with DHEA n 70 ; vs placebo n 70 ; , modest increases in libido and bone mineral density were observed, but only in the subgroup of women older than 70 years of age 49 ; . There are very consistent reports of significant decreases in HDL cholesterol following DHEA replacement. Two recent studies reported an improvement in some parameters of insulin sensitivity following 12 weeks of DHEA replacement in adrenal insufficiency 50 ; and six months of DHEA in elderly women 51 ; . The latter study also reported a significant decrease in fat mass as assessed by magnetic resonance imaging MRI ; in 14 women treated for six months 51

Educate management on the issue: Provide executives, supervisors, and human resources staff with information about: a ; CSHCN, b ; the physical and emotional impact of caregiving on parents, and c ; the special problems employees with very sick children face as they juggle home and work responsibilities. Provide education and support, when feasible: Create opportunities for employees who have children with special needs to gain support from each other. Provide employees with information on local support groups. If there is sufficient demand at the worksite, consider launching a support group by providing meeting space at a company location. Conduct seminars in the workplace after hours ; for families of children with special needs on topics such as financial planning, finding appropriate childcare, and managing stress, or refer families to community resources. Summary Points Well-child care is preventive health care for children and adolescents. One of the primary purposes of well-child care is to identify children affected by a physical, mental, or developmental problem as early in life as possible. All children face health risks; yet, many child health problems are preventable. Child and adolescent illness and injury are a major cause of employee absence and lost productivity. Employers have opportunities to reduce preventable health problems through benefit design, communication, and employee education. Children with special health care needs are an important part of an employer's beneficiary population. These children experience complex, chronic, and severe health problems, which can be difficult to manage; they use more healthcare services than other children and thus have higher overall healthcare expenditures; and they experience more sick days than other children, which results in lost productivity and absenteeism for their parents. Employees with sick children who receive help and support from their employers are usually better able too concentrate on their jobs, and remain with their companies longer. Employee retention is a key driver of customer retention, which in turn is a key driver of company growth and profit. Improving the health of children will likely benefit an employer's bottom line by reducing both direct healthcare costs and indirect costs, such as lost productivity and miglitol.
COMPARISON OF CHRONIC DISEASE SCORES CDS ; AND MEDICATION-BASED SCORES MBS ; IN MEMBERS UTILIZING COX-2 INHIBITORS AND NSAIDS FROM A PHARMACY BENEFITS MANAGEMENT SETTING Marjan Massoudi * , Todd Sauer Walgreens Health Initiatives, 1417 Lake Cook Rd, MSL457, Deerfield, IL, 60015 marjan.massoudi walgreens The purpose of the study was to determine if members identified in a pharmacy claims database as chronic COX-2 Inhibitor utilizers attain a higher Chronic Disease Score CDS ; compared to chronic NSAID utilizers. In addition, CDS was compared to medication-based scores MBS ; to evaluate the validity of the scoring tool. This study was a retrospective claims database analysis conducted in order to identify members who had received at least 90 consecutive days of COX-2 Inhibitor or NSAID prescriptions defined as chronic utilizers ; between 7 01 and 7 01 02. Chronic utilizers who were identified as having a 30 day period of prior COX-2 Inhibitor or NSAID prescription use were not included in the study. Members were evaluated by using a Chronic Disease Score CDS ; instrument, which determined each member's chronic disease status based on empirically-derived weights from prescription utilization, age, and gender. The CDS analysis was compared to MBS, which were determined by counting the number of unique prescriptions for each member. 427 COX-2 Inhibitor and 131 NSAID chronic utilizers were identified for the CDS and medication-based score analysis. Pharmacy claims analysis will be presented. Learning Objectives: Determine the Chronic Disease Score CDS ; and medicationbased score MBS ; from pharmacy claims data. Determine the validity of the CDS instrument as a scoring tool. Self Assessment Questions: The CDS instrument is a reliable predictor of co-morbidity in claims database research. And the inhibitory effect of VEGF on sema 3A-induced chemorepulsion was blocked by EG3287. These findings indicate that inhibition of basal DRG neuronal growth cone collapse and VEGF antagonism of the chemorepellent activity of sema 3A are NP-1-mediated functions. The finding that EG3287 markedly increased basal growth cone collapse in the absence of exogenous VEGF or sema 3A suggests that binding of this peptide may also inhibit constitutive functions of NP-1 that are essential for maintaining growth cone integrity, a possibility that warrants further investigation. The biological functions of PlGF isoforms are poorly understood, and effects of PlGFs in primary neuronal cells have not previously received much attention. The finding that PlGF-2 had a marked anti-chemorepulsive effect in DRG neurons, similar to the effect of VEGF, indicates that NP-1-binding PlGF isoforms can elicit biological effects independently of Flt-1, and the finding raises the possibility that PlGF acting through NP-1 may have neurotrophic properties in vivo. The most well established biological role for NP-1 is in mediating chemorepulsive effects of sema 3A in neuronal axons, but because NP-1 recognizes a variety of semaphorins that display biological effects antagonistic to those of sema 3A and is also able to bind VEGF, a potent chemoattractant, it is likely that the function of NP-1 is not restricted to chemorepulsion in sensory neurons. An implication of the present study is that NP-1 is a bifunctional receptor capable of mediating either chemorepellent activity or neurite outgrowth depending on the stimulating ligand. Neuropilins are not thought to be sufficient to relay a biological signal alone, partly because of the lack of well defined signaling motifs in their cytoplasmic domains. Interaction between NP-1 and plexin A is required for sema 3A-triggered growth cone collapse 42 ; , although identification of the post-receptor signals involved in mediating the chemorepellent activity of sema 3A has so far proved elusive. Because VEGF and PlGF-2 prevented growth cone collapse via NP-1 in the absence of KDR and Flt-1 receptors, it is unclear whether these ligands are acting to inhibit NP-1-mediated collapse and or are stimulating an alternative signaling pathway via NP-1 that promotes neurite outgrowth. The fact that VEGF stimulated growth cone activity in the absence of other exogenous factors suggests that NP-1 may mediate a VEGF signaling response leading to inhibition of growth cone collapse, but both the nature of this response and how VEGF binding to NP-1 is able to transduce a biological signal in DRG neurons remain to be determined. Candidates for mediation of NP-1-dependent VEGF DRG neuronal signaling are plexins, and a recently identified PSD-95 Dlg ZO-1 domain protein, NP-1-interacting protein-1 synectin or RGS-GAIP-interacting protein ; , binds to a carboxyl-terminal PSD-95 Dlg ZO-1 domain binding motif in NP-1 43 ; . The results of this study also reveal a previously unknown role for COX-derived prostanoids in mediating anti-chemorepulsion in DRG neurons. IP receptors for prostacyclins and EP receptors for PGE1 and PGE2 are strongly expressed in DRG neurons 39, 44 ; , and prostacyclin analogues and PGE1 and PGE2 stimulate a variety of responses in sensory neurons, including adenylyl cyclase activity, cyclic AMP production 45 ; , sensitization to bradykinin 46, 47 ; , and neuropeptide release 39, 45, 47 ; . COX-derived prostanoids are thought to have important physiological roles in the mediation of pain and inflammation 48 ; , and IP receptor-deficient mice display impaired nociception, anti-thrombosis, and inflammatory responses 38 ; . VEGF induces PGI2 production in endothelial cells via extracellular signal-regulated kinase-mediated activation of cytosolic phospholipase A2 12 ; and induction of COX-2 49 ; , and PGI2 and other prostaglandins have been implicated.
