In this study we identify a novel soluble DS proteoglycan secreted by endothelial cells, which promotes HGF SF-induced proliferation through its glycosaminoglycan chain. Based on the restricted expression of this proteoglycan to endothelial cells, we propose to call it endocan. LRP-200306-03 A Review of the Evidence for the Effectiveness, Safety, and Cost of Acupuncture, Massage Therapy, and Spinal Manipulation for Back Pain. D. C. Cherkin, K. J. Sherman, R. A. Deyo, P. G. Shekelle. BACKGROUND: Few treatments for back pain are supported by strong scientific evidence. Conventional treatments, although widely used, have had limited success. Dissatisfied patients have, therefore, turned to complementary and alternative medical therapies and providers for care for back pain. PURPOSE: To provide a rigorous and balanced summary of the best available evidence about the effectiveness, safety, and costs of the most popular complementary and alternative medical therapies used to treat back pain. DATA SOURCES: MEDLINE, EMBASE, and the Cochrane Controlled Trials Register. STUDY SELECTION: Systematic reviews of randomized, controlled trials RCTs ; that were published since 1995 and that evaluated acupuncture, massage therapy, or spinal manipulation for nonspecific back pain and RCTs published since the reviews were conducted. DATA EXTRACTION: Two authors independently extracted data from the reviews including number of RCTs, type of back pain, quality assessment, and conclusions ; and original articles including type of pain, comparison treatments, sample size, outcomes, follow-up intervals, loss to follow-up, and authors' conclusions ; . DATA SYNTHESIS: Because the quality of the 20 RCTs that evaluated acupuncture was generally poor, the effectiveness of acupuncture for treating acute or chronic back pain is unclear. The three RCTs that evaluated massage reported that this therapy is effective for subacute and chronic back pain. A meta-regression analysis of the results of 26 RCTs evaluating spinal manipulation for acute and chronic back pain reported that spinal manipulation was superior to sham therapies and therapies judged to have no evidence of a benefit but was not superior to effective conventional treatments. CONCLUSIONS: Initial studies have found massage to be effective for persistent back pain. Spinal manipulation has small clinical benefits that are equivalent to those of other commonly used therapies. The effectiveness of acupuncture remains unclear. All of these treatments seem to be relatively safe. Preliminary evidence suggests that massage, but not acupuncture or spinal manipulation, may reduce the costs of care after an initial course of therapy. Unfortunately, not even this measure provides a valid basis for age comparisons since the ratio of licensed drivers to active drivers may differ greatly among age groups, especially if different legal measures, such as health controls or driving tests, or differing access to automobile insurance, are applied at a certain age. Screening measures usually make it less plausible for nondrivers with increasing age to take the trouble of continuing to be licensed, which gives these age groups a worse crash-to-license ratio. In addition, active driver groups of comparable sizes have dissimilar exposure to crashes if their yearly mileage differs. To overcome this source of error in estimating driver safety, it is generally believed that a hard estimate of the actual amount of driving e.g., miles driven ; is needed. Unfortunately again for the validity of the findings, the relationship between yearly mileage and crash rates per mile is not linear but curvilinear, so that high-mileage drivers have, independent of age, lower crash rates per mile than lowmileage drivers. Hence, older drivers' risk estimates based on miles driven are overestimated when compared with those of younger drivers having a higher yearly mileage 16 ; . Moreover, there is no available adjustment for the degree of difficulty of the roads older drivers choose to use. In conclusion, none of the measures presented in Figure 1 permits straightforward conclusions about how drivers' individual risk of crashes increases with age.

For this purpose, we develop a conceptual model including memory effects and using precipitation and air temperature data as input variables. We show in terms of correlation functions that this model describes main dynamical properties of the runoff, especially the delay between rain event and runoff response as the annual period in the runoff data. On the other hand, we apply the ACE Alternating Conditional Expectation ; algorithm, in order to get a nonlinear regression model that simulates the runoff data. We discuss the results of both models and their relation and procaine.
In a prospective, double-blind, randomized, placebocontrolled study we investigated the facilitating influence of intravenous procainamide on conversion of atrial flutter by rapid atrial pacing. Fifty consecutive patients with spontaneous sustained atrial flutter were 1: randomized into two homogenous groups: group A received lOmg " 1 procainamide intravenously, group B placebo. | Effects of Griseofulvin on Seeded MT Assembly. MT nucleating seeds were constructed by polymerizing goat brain tubulin 3.5 mg ml ; in the presence of 6 M glycerol and 1 mM GTP at 37C for 45 min and shearing the MTs into small fragments by repeated passage through a 25-gauge needle. For measurement of polymer level, desired griseofulvin concentrations were first incubated with 10 M tubulin at 37C for 10 min. The MT seeds were then added a 1: 5 ratio of tubulin in the suspension of seeds to soluble tubulin; final tubulin concentration of 12 M ; Polymerization was started by adding 1 mM GTP and incubating at 37C for 45 min. SUMMARY We measured myocardial blood flow and myocardial procainamide concentration in 24 sections of left ventricle following occlusion of the left circumflex coronary artery in 11 dogs. "C-Labeled procainamide was infused at a constant rate 40 jig kg P e following completion of the coronary artery occlusion. At the end of 4 hours, the infusion was terminated and the dogs were killed. Regional myocardial blood flow was determined at 30 minutes and at 4 hours, using I2SI- and 5 Cr-labeled microspheres. Regional myocardial procainamide concentration was determined by analysis of "C content in the samples, using a combustion technique with entrapment of liberated I4 CO 2 The amount of 14C activity in each sample was calculated following corrections made for 51Cr and I25I activity, using the channels ratio method. Twenty-four regional sections of left ventricle were divided into 12 anteroseptal control sections and 12 posterolateral "ischemic" sections. Myocardial blood flow to the control region was 1.17 0.17 mean SEM ; ml min per g and myocardial procainamide concentration was 4.62 0.36 ig g. Plasma procainamide concentration was 2.2 0.1 Mg m'- Flow to ischemic sections, when measured at 30 minutes and 4 hours, did not change. Myocardial procainamide concentrations in the ischemic sections differed significantly from procainamide in the control sections only when flow fell to 31-40% of control values P 0.01 ; . At flows less than or equal to 10% of control flow, myocardial procainamide concentration was reduced to only 42 4% of control drug levels. We conclude that in our experiments there is a substantial concentration of procainamide in severely hypoperfused and probably densely infarcted ventricular myocardium.

Background--Brugada syndrome is a form of idiopathic ventricular fibrillation characterized by a right bundle-branch block pattern and ST elevation STE ; in the right precordial leads of the ECG. Sodium channel blockers increase STE. Mutations of the cardiac sodium channel SCN5A cause the disorder, and an implantable cardioverter-defibrillator is often recommended for affected individuals. Mutations in other genes have not been identified, and it is not known if the efficacy of drug testing or the malignancy of arrhythmias correlates to the gene defect. Methods and Results--We performed histories, physical examinations, ECGs, and drug testing on a large multigenerational family with Brugada syndrome. DNA isolated from blood samples, polymorphic genomic markers, and polymorphisms within candidate sodium channels were used for a genome-wide screen, fine mapping, and linkage analysis. We identified 12 affected individuals right bundle-branch block, 1-mm STE ; with an autosomal dominant inheritance pattern characterized by incomplete penetrance that appeared to be dependent on age and sex. Four affected individuals had syncope and 2 had documented ventricular arrhythmias, but there was minimal family history of sudden death. Procainamide infusions did not identify additional affected individuals. Linkage was present to an 15-cM region on chromosome 3p22-25 maximum LOD score 4.00 ; . The sodium channel genes SCN5A, SCN10A, and SCN12A on chromosome 3 were excluded as candidates LOD scores 2 ; . Conclusions--A Brugada syndrome locus distinct from SCN5A is associated with progressive conduction disease, a low sensitivity to procainamide testing, and a relatively good prognosis in a single large pedigree.
